Dr Sebastian is not accepting new patients at this time. Fees for appointments for established patients are prorated at $200 per hour. Schedule a 30 minute or 60 minute appointment based on the amount of time that you may need. The actual fee is based on the amount of time spent during the appointment.
Insurance – Dr. Sebastian does not currently participate as a provider for any private insurance companies or Medicare or Medicaid. For all questions regarding insurance, you should call your insurance company and ask if they will reimburse telemedicine consultation fees for an out-of-network provider.
Medicare patients: If you have Medicare, you are required to sign a contract indicating that you are aware that fees for the consultation will not be reimbursed.
