Appointments for new patients are scheduled for 60 minutes unless other arrangements have been made in advance.
Fees – For a new patient, the fee is $250 for a 60-minute appointment. If more time is needed, the fee will be increased accordingly. For follow-up visits, the fees are prorated at $200 per hour with a minimum fee of $75. Payments may be made online after each consultation.

Insurance – Dr. Sebastian does not currently participate as a provider for any private insurance companies or Medicare or Medicaid. For all questions regarding insurance, you should call your insurance company and ask if they will reimburse telemedicine consultation fees for an out-of-network provider.
Medicare patients: If you have Medicare, you are required to sign a contract indicating that you are aware that fees for the consultation will not be reimbursed.
